Client questionnaire vs. client intake form: what's the difference?

Use an intake form to qualify and scope. Use a questionnaire to understand. The intake form tells you whether the project is viable because it covers budget, timeline, and deliverables. The questionnaire tells you how to approach it because it covers brand positioning, past agency relationships, decision-making culture, and the nuances that shape how the work should be done.

In practice, many agencies combine both into a single pre-kickoff document. That works well if the questions are grouped clearly. What you want to avoid is asking understanding questions before you have confirmed the project is scoped and budgeted, because it signals you are more interested in the work than in whether it is set up to succeed.

  • Who has final approval on creative, copy, and technical deliverables?
  • How are decisions typically made on your team, by consensus or by a single decision maker?
  • Is there anyone who needs to be consulted but does not have formal approval authority?
Content and assets
  • Who is responsible for providing copy, images, and other content?
  • Do you have existing brand guidelines, a design system, or a style guide?
  • What content or assets from your current setup should be preserved in the new work?
Competitive landscape
  • Who do you consider your top three competitors?
  • How do you want to position against them in this project?
  • What do your competitors do well that you want to differentiate from?
